First Visit
Your initial appointment will consist of a thorough examination and completion of required diagnostic testing (radiographs, etc.) to properly determine your current dental/oral health status. Occasionally, treatment can commence the same day as the examination/consultation. However, a complex medical history and/or treatment plan will require a second appointment to properly consult with you and determine an appropriate treatment plan schedule.This process is necessary in order to keep you, the patient, thoroughly informed as to every aspect of your care.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any x-rays (if applicable)
- A list of your current medications
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
X-Rays
If your prior dentist has taken current x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
Should x-ray copies provided by your previous office be of poor quality or are not up to our diagnostic standard, we will not accept them as part of your file. If new or additional films are determined to be needed by your doctor, they can be taken at our facility and will incur our standard x-ray fee.
